The concept of Synergy can be applied to the practice of nursing by means of education and service. To obtain Synergy in nursing the nurse’s knowledge and skills are able to meet the individual needs of the patient population that is presented. Currently in Western Pennsylvania, there is a rising number maternal substance abuse during pregnancy. This epidemic in turn creates a different type of population in infants than other areas of the Neonatal Intensive Care Unit (NICU), with specific healthcare needs. One area that is lacking synergy is the care of this rising patient population of infants suffering from Neonatal Abstinence Syndrome (NAS). This creates a gap between nursing education and clinical application. Due to the rise in NAS infants it is important for nurses to be educated in proper care for an infant presenting with this diagnosis, symptom management, communication skills, and the fundamental knowledge Neonatal Abstinence Syndrome. Poor patient outcomes can be caused by a lack of understanding addiction. The Vermont Oxford Network states, â€Å"Fear of stigmatization, discrimination, child removal, poor treatment, and criminal prosecution has deterred women from seeking care. However, the research suggests that providing substance dependent women with comprehensive healthcare, drug and alcohol abuse treatment, and social support improves pregnancy, birth, and child development outcomes. † (Horbar, Soll Buus-Frank, 2013) When the infant is admitted into the acute care setting, the care needs of these patients should focus less on the immediate outcome, and more on what will create the greatest good for each patient as an individual. There is an essential need for the development of collaborative solutions for care and education in the healthcare setting for the care of this rising population. Often this would be to provide care with a realistic goal of maintaining optimal levels of comfort with minimal side effects of withdraw. â€Å"About 40% of infants who exhibit withdrawal symptoms can be treated without medication. † (White, 2013) Nurses who take time to ensure a level of comfort will help to alleviate symptoms of NAS withdraw and also help support the family in their infant’s care. In addition to providing comfort will help decrease the need to be medicated for withdraw and ultimately will lead to a shorter hospitalization. The manager of a Neonatal Intensive Care Unit will need to address this issue and implement change in order to improve patient outcomes. The competencies that make up the Synergy model are clinical judgment, advocacy and moral agency, caring practices, facilitation of learning, collaboration, systems thinking, response to diversity, and clinical inquiry. For the NAS population advocacy, clinical judgment, caring practices and facilitation of learning are most important. Advocacy proposes the nurse to serve as a moral agent to work on the infant’s behalf in order to provide the greatest good to help resolve concerns that are both clinical and ethical. Clinical judgment is the ability for the nurse to take on the leadership role to care for their patient. Infants are unable to make healthcare decisions; the nurse must consider the parent’s wishes and what the result the nurses’ actions will yield for this infant. These actions must positively contribute to their plan of care overall. Caring practices creates a compassionate and therapeutic environment for each the patients, recognizing the unique needs of each infant. The nurse acts as a facilitator of learning when he/she incorporates the understanding of the infant’s family into the care. The Synergy mode includes patient characteristics, which are, resiliency, vulnerability, stability, complexity, resource availability, participation in care and decision-making and predictability. For the NAS population the patient characteristics would immediately associate complexity, vulnerability, and participation in care and decision-making. Complexity as referred to as to the multitude of symptoms that are to be considered for proper care of the patient. These infants are unable communicate pain, symptoms needs, and what their desire’s are. For example, these infant’s cry and are inconsolable until the nurse goes through a trial and error until he/she figures out the needs of the child. They do not understand what is happening to them. Vulnerability includes the stressors that these patients will be exposed to that may affect outcomes. The infants depend on the decisions their families and healthcare professionals. Increased length of hospital stay can lead to problems that may decline in their already compromised condition, for example, infection. Lastly, the NAS patients are vulnerable due to the fact that they are unable to make their own healthcare decisions and have to depend on parents and healthcare providers to be speak on their behalf. The parents participate in care as they receive information and help assist in the care for their child. Analyze the Case Nursing education places emphasis on clinical presentation, physical stressors and conditions that affect the patient and their associated treatments. Increased education is needed for nurses on the postpartum and NICU units about the treatment for this specific patient population. With the increase in drug dependent mothers there seems to be little nursing education provided on postpartum and NICU nursing units about the care for these infants, and this issue needs to be addressed. â€Å"Infants with NAS have higher rates of neonatal complications, prolonged lengths of stay and consume substantial NICU and hospital resources. Further, they impose a growing burden on already strained healthcare resources because state Medicaid expenditures are disproportionately impacted. † (Horbar, Soll Buus-Frank, 2013) The administration and education team needs to identify this problem and incorporate teaching and updated research on these units. One competency that lacks synergy in the nursing care of the NAS population is collaboration. â€Å"Collaboration enhances the capacity of a group and increases the potential for success. † (Hardin Kaplow, p. 75, 2005) It is not uncommon when different discipline areas such as the nurse, doctor, social worker etc. identify different goals for the patient. Poor communication causes the team to not work in unison to collaborate and work towards a common goal. â€Å"A key to collaboration is the communication that must exist between the patient, family, and health care team members as well as among members of the health care team. Another synergy lacking competency would be caring practices, which is the unique nursing care provided to the patient and families. Within the NAS population nurses have to recognize the differences and incorporated them into the infants care. Having compassion and creating a calming environment help provide comfort in a therapeutic environment. Often times the NAS population is considered as level II care, this allows for the nurses to take on a multiple patient assignment of three to five infants at a time. The NAS population is unique; they require much coddling and almost constant attention from the nurse. Having greater than three patients causes a great disadvantage to these children due to the nurse having to spread herself thin to meet the basic needs of feeding, changing and medicating, the nurse does not have adequate time to rock and soothe the infants causing greater withdraw symptoms. The rational decision-making model will help identify and implement the appropriate changes that are essential in optimizing care for the NAS population. Improvement in the education of the healthcare staff will create better patient outcomes. This decision making process includes three steps to achieve this objective: identify possible outcomes, determine probability of each outcome, and take action with the highest probability to achieve a positive outcome. The considered actions are increased in-person training, mandatory review of printed materials, or an on-line training course. The greatest positive outcome would come from increased in-person training. Create a Change Proposal Part of the nursing orientation for staff will be to participate in a class specified for the training related to care of the NAS infant prior to working in the NICU. Written material as well as a presentation will be provided. The attending physician will make a statement about services provided to the NAS infant, medications and important facts that the physician would like in report. For the staff nurses’ senior nursing projects are assigned yearly and are presented to maintain their status on the unit. Assigning NAS care to a few nurses each year will keep the most up to date research on the unit. These nurses will participate in Internet presentations that focus on quality, safety and care for infants and families of and NAS infant. Finally, a staff member from the social work team will join and present his/her role in the case of a NAS infant. The unit will provide CEU’s for this training. This proposal will affect the unit internally by involving the management team and multiple staff disciplines. The management team will help aid in this proposal by providing a day that is designated this additional training and appoint the appropriate nursing, physician and social work staff to participate in running the program. Finally obtaining CEU’s approved by the state board of nurses to offer education credits for after the completion of training. The designated nurses assigned to NAS as a senior project must keep up to date with the training information prior to the class of new staff, doing so will ensure they are prepared for questions that may arise. With this proposal the staff has the potential to increase the patient stability, outcome and satisfaction. Also this proposal will be aimed to help the nurses caring for NAS infants to promote patient care through evidenced based research. A change model that applies to this proposal would be the Lewin’s change model. This model is based on unfreezing, moving, and refreezing. First the unfreezing stage, the current beliefs and ideas will change with the desired outcome. The health care team will sufficiently identify the epidemic and need for changes in care for the NAS infant, inspiring the education and treatments needed to move the staff towards bettering practices. Lastly, refreezing, the nursing staff will have a new outlook or attitude towards NAS infants. Changes in the treatment process will be instituted based on the staff’s new outlook. Having the knowledge and resources for the care of the NAS infant will allow the care team to handle each unique case with evidenced based approach. Along with any type of change there will always be positive and negative outcomes on the environment. Positive outcomes will include an influx in nursing knowledge, better patient outcomes and an increased amount of resources for the staff. Negative outcomes may include not having the adequate amount of staff to run this program and the biases formed by individuals prior to participating in this class. A way to follow up with the care provided to these patients and their families would be an optional survey completed by the families allowing the unit to see their strengths and weaknesses. The survey will allow for improvement of care. â€Å"Nurses who have confidence in their ability to positively impact maternal parenting behaviors through education and skill building may impact patient care outcomes. It is imperative that the management and supervisors recognize the issues that nurse’s experience when caring for NAS infants. Educating staff will allow for a balance of practice and the nurses actions. Instituting this educational program will provide synergy to the NICU units and the epidemic of withdraw infants. The unit directors will be able to see that the changes to the unit and education instituted has worked when NAS infants have a shorter hospital stay, a decrease in the use of morphine and phenobarbital and finally patient/parental and nurse satisfaction. In conclusion, with the epidemic of substance abuse mothers on the rise it is imperative that neonatal nursing units properly educate their staff about the treatment and care of NAS infants. With the changes proposed in this case the care of this specific population will be enhanced. These infants are very fragile and require a lot of special attention. The sale and distribution of the plastic rings should begin immediately after to all of their branches so that way PWI can start earning profits as quickly as possible. PWI currently has a specialized inventory because the steel rings that they produced were made from a unique type of steel. There would be sunk costs that would ultimately come from the failure of PWI to sell back the specialized steel because of the same features that make the steel unique would in the end be the reason that they are difficult to resell. An Overview of the Book Democracy in America Democracy in America, written by Alexis de Tocqueville between 1835 and 1840, is considered one of the most comprehensive and insightful books ever written about the U.S. Having seen the failed attempts at a democratic government in his native France, Tocqueville set out to study a stable and prosperous democracy in order to gain insight into how it worked. Democracy in America is the result of his studies. Unlike Miss Emily Annie didn’t get away with what she did to Paul, after killing the officer Paul tricks her into buying Champagne and cigars to celebrate the return of Misery, while setting up for their night of celebration Paul lights the re-written novel on fire and throws it. Annie begins to put out the flame when Paul slams his typewriter on the top of her head and shoves pieces of the burned novel pages into her mouth, she then dies of a fractured skull. Sometime after Paul is rescued by police. Both of these stories where great and though they are in different settings the characters shared similar physiological problems and depression. I think if both of these women met and could ever be in the same story there would be gory, suspenseful, torturing of men. I would like to read a book where Stephen King and William Faulkner write a story sharing both their ideas, I think it would be very interesting, thrilling and great to read.

Cultural Anthropology and Ethnographic Fieldwork James P. Spradley (1979) described the insider approach to understanding culture as "a quiet revolution" among the social sciences (p. iii). Cultural anthropologists, however, have long emphasized the importance of the ethnographic method, an approach to understanding a different culture through participation, observation, the use of key informants, and interviews. Cultural anthropologists have employed the ethnographic method in an attempt to surmount several formidable cultural questions: How can one understand another's culture? How can culture be qualitatively and quantitatively assessed? What aspects of a culture make it unique and which connect it to other cultures? If ethnographies can provide answers to these difficult questions, then Spradley has correctly identified this method as revolutionary. Cultures are infinitely complex. Culture, as Spradley (1979) defines it, is "the acquired knowledge that people use to interpret experiences and generate social behavior" (p. 5). Characteristics of Malcolm Rivers Essay

Which is more powerful, the human mind or body? And, how can a mental illness like dissociative identity disorder effect the way we connect our mind and physical body? Malcolm Rivers, a convicted murderer in the movie Identity, is on death row, awaiting his execution for several brutal murders he had orchestrated. Due to that fact that Rivers had been diagnosed with a mental illness, D. I. D, he has no control and did not receive any medical/psychological help, he cannot be held accountable for his actions. In the movie, Malcolm has blackouts which sometimes last for days at a time. These blackouts cause him to have no control over his mind or body. He believes that he is one of the characters that have been created in his head. An example of this would be when Dr. Mallick hands him the mirror to see his reflection, Rivers does not expect to see what he saw, he was expecting to see who he thought he was at the time (the cop). This shows that he has no control over who he thought he was. This also relates to control over his actions, if in his mind he was not who he believed to be, then he cannot be responsible for the actions portrayed by a fake internal personality. A second example which shows that he has no control, is when he fades in and out of character at random times. During the same mirror scene, Malcolm bizarrely fades back into the internal personality of the cop. There is no mental or physical control over these fade backs, leaving Mr. Rivers helpless in the situation. Mr.  Rivers was not offered any medical treatments or help, meaning, Malcolm was helpless; he was definitely suffering mentally caused by many traumatic incidents he was put through as a child. In the beginning of the movie, for example, we hear (from the tape recorder) Dr. Mallick asking Malcolm about his childhood. The things that had been said about his mother being a prostitute and stories about his childhood where he was left at a motel, are sure to cause any child psychological trauma, and with no one to confide in or to speak to about his problems, he was bound to get D.  I. D; this mental disorder proves that he cannot be responsible for the murders. Dr. Mallick only helped to prove that it was the ten personalities controlling Malcolm’s actions, and not Malcolm himself, but he failed to help him seek treatment or psychological help before any of this happened. If only Malcolm had been given the attention he needed, he would not have built up all this repression. Furthermore, the last personality, timmy, who is like a reflection of his brutal upbringings as a child, turns out to be the real murderer. Malcolm Rivers is not responsible for the murders and should not be held accountable for any negative actions done while his state of mind is in that of one of the internal personalities. Mr. Rivers is unable to control his mind and body; shown when he has uncontrollable black outs and fade backs into one of the made up personalities. Malcolm is not mentally stable and probably has post traumatic stress syndrome; He has not received any real form of psychological help, which is another reason as to why he is so helpless, and therefore not responsible

You’ll notice that key among her findings was how a headline either had little to do with the post (and people bounced out as soon as they realized it) or was a dog from the get go. Headlines, headlines, headlines. So important. I read Dodaro’s entire blog post because I’m like most of you: I have a slight penchant for schadenfreude mixed with curiosity over the confessional approach. So that, right there, is a great way to turn blog post duds into traffic getters. 5. Your Practice Is Never Wasted Practice doesn’t make perfect, but it does make a habit. Not only did that bad content still have a role in your writing training and habit-making, it fulfills that important role of letting you experience failure so you know better what is good, what is bad, and the art of being able to pick yourself up and move on. Everything- even the failures- is a data point. Hopefully you’re tracking it. 6. It Might Be Time To Shift Your Strategy Perhaps your content wasn’t all that bad, but it performed badly because your social sharing, paid promotion, email, or other promotion methods are in need of a tune-up. Are you promoting your content at the wrong time of day or the wrong day of the week? Has your audience changed since when you first started? Are you on the wrong social network? Do you need to send out more emails? You’ll often see blog post suggesting that you review your data, your results, and ultimately your strategy periodically. If you’ve noticed a series of poorly performing content, it might be a sign you need to do that, that your strategy needs to be retooled. You've had blog posts bomb. How often do you check your data to know what really happened?7. You Didn’t Leave Enough Time Maybe you slipped a bit and weren’t doing as well as you’d hoped as far as planning your content on your editorial calendar. You didn’t give yourself enough lead time to keep up with posts, and are fast approaching the point where you’re behind the curve instead of ahead of it. What do you do in a rush? You start to lose sight of important things, such as keeping your SEO tight and your copy audience-suited. Not all poorly-performing content is the result of letting time get away from you, but it’s worth an honest look. This is one of the reasons an editorial calendar is so vital. It keeps you from falling into that dark valley of â€Å"OMG, I need a post for tomorrow!†, a valley where sloppy and shallow are most likely to emerge. Look at the poor performing content, and take a look at your editorial calendar or work schedule. How much of a rush were you in? If you had a post that bombed, how fast did you write it? You can use that idea again, here's how.If you can’t keep up with the content quality because you bumped the quantity up a few notches, bring it back down and work on focusing on a singular post a week. 8. Your SEO Has Gone Wild Being rushed sometimes means your SEO is barely given a nod. On the flip side, though, getting too narrow a focus on a keyword can also derail your content. Dodaro admits as much for one of her failed blog posts, noting that the problem with a post was a faulty headline which had an â€Å"SEO keyword crammed in at the expense of title’s integrity†. Yep. We’ve done that, too. Ever see those weird headlines that look like someone bungled the grammar, leaving out the article in the midst of a keyword phrase or seeming slightly off from how a real person talks? â€Å"How Market Research Firm Can Help You† â€Å"What Time Is The Super Bowl?† (An actual headline from the Huffington Post. Probably a huge long-tail search term, but how much content can you really create for that without seeming obnoxious?) â€Å"The Business Social Media Hacks That Work† That’s what happens when your headline is an indentured servant to keywords and SEO. It’s as if you’re playing the Cat Game from Super Troopers, packing keywords everywhere you can: Fast Company puts this well: â€Å"If you find yourself trying to fit a square-pegged keyword into a round-shaped title, just let it go. You don't want to write something that doesn't make sense or confuses the reader just for the sake of SEO.† Clunky headlines that search engines might love but people ignore don’t do you any favors on social media and in email. 9. Decide If You Need To Cull It I won’t recommend deleting content, but if you are unable to bear the thought of that awful content existing, perhaps it’s time to remove it. However, be sure to salvage what little traffic you had coming to it. Direct that low-performing post to a brand new, much better post using a 301 redirect. Or, follow the Moz blog’s suggestions for weeding out low-performing content and cleaning up your site. 10. Look For Salvage Maybe you can save it. How bad was your post? Be sure you are really identifying your site’s worst-performing content and not just the stuff you don’t like yourself. At that point, there are likely things you can still salvage if the sum of the parts happened to be less than stellar. Did you have an idea for an interview and the interview went badly? Maybe the interviewee answered in one- or two-word answers. Can you turn that interview into data or other forms (charts, percentages, or recharacterizations)? Maybe you got sloppy and cheated a bit on the headline by using the first one that came to mind instead of really working through the best options.Why not change the headline, and reset your social sharing and promotion to see if it fares better? At the very least, you can get some A/B testing data out of that between the two headlines. Take Action:  Look through your analytics and choose at least three of your worst-performing blog posts. Dissect them. Figure out why they bombed. Was it: Headline problems Copy failures Wrong topic Wrong timing Lack of proper promotion Uninterested writer Dissect your bad content and figure out why it  didn’t work. Rework them into a new piece of content, and try again, taking into account the things you did wrong the first time. Or, develop a checklist for yourself to be sure you don’t repeat the same mistakes for future content.
